Service options include in-home care and rehabilitative services, companion services, housekeeping, meal preparation/delivery, maintenance and lawn services, transportation and much more.

• Your care manager will meet with you and your family in the home to evaluate existing abilities and needs.
• Your care manager will develop a plan to address all needs identified, including companion care, community services and adult day services.
• Through visits and phone contacts, your care manager monitors care, provides support and stays informed.
